Your Rights
- Receive home health services free from discrimination, harassment, or retaliation.
- Request reasonable accommodations or auxiliary aids to communicate effectively.
- Get information about your care in your preferred language.
- File a grievance if you believe your civil rights have been violated.
Language & Communication Assistance
We offer language assistance services at no cost to the patient. Contact us if you need documents translated or an interpreter for appointments. For clients requiring TDD or TT devices, we coordinate with the Illinois Telecommunication Access Corporation (ITAC) to arrange communication support services.
How to File a Complaint with CMHH
