About Our Services
Home health care brings skilled medical services—like nursing, physical therapy, and wound care—directly to your home. It's designed for patients who need professional care but don't require hospitalization.
We provide skilled nursing, home health aide services, physical therapy, occupational therapy, speech therapy, and medical social work. We can also coordinate additional support services like durable medical equipment and meals.
Our services are typically scheduled during daytime hours, with visit frequency based on your physician's orders and your individual care plan. We do not provide around-the-clock care.
Getting Started
Call us directly at 847-784-9966, request a call through our website, or ask your physician to refer you. We'll schedule a free in-home assessment to discuss your needs.
Yes. Medicare and most insurance plans require a physician's order to begin home health services. If you don't have a referral yet, we can help coordinate with your doctor.
We typically schedule an initial assessment within 24-48 hours of receiving a referral. Services can often begin within a few days, depending on your needs and availability.
Insurance & Payment

We offer private-pay options for patients without insurance coverage. Contact us to discuss rates and payment arrangements.
Caregivers & Staff
Yes. All of our clinicians (nurses, therapists, social workers) are licensed by the State of Illinois. Our home health aides are certified and work under nursing supervision.
We do our best to provide consistency by assigning the same clinician(s) to your care. Occasionally, scheduling or coverage needs may require a different team member.
