Registered Nurse (RN)

Job Overview

Chicago Midwest Home Health is seeking a compassionate and skilled Registered Nurse to join our team. You'll provide high-quality nursing care to homebound patients across Chicagoland, working independently while being supported by our experienced clinical team.

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, wound care, IV therapy, and other skilled nursing interventions
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on disease management and self-care
• Coordinate care with physicians, therapists, and other healthcare providers
• Maintain accurate and timely documentation in compliance with regulatory standards
• Supervise home health aides and provide clinical guidance

Required Qualifications

• Current Illinois Registered Nurse (RN) license in good standing
• Minimum 1 year of clinical nursing experience (home health preferred)
• Valid driver's license and reliable transportation
• Current CPR certification
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Proficiency with electronic medical records (EMR)

Preferred Qualifications

• BSN degree
• 2+ years of home health or community nursing experience
• OASIS-D1 certification
• Bilingual (English/Spanish)

Work Schedule

Monday - Friday, flexible hours. Weekend and on-call rotation as needed. Typical caseload: 5-7 patients per day.
