Physical Therapist (PT)

Job Overview

We're seeking a licensed Physical Therapist to deliver in-home PT services to help patients regain mobility, strength, and independence. You'll work with a diverse patient population while enjoying flexible scheduling and professional autonomy.

Key Responsibilities

• Conduct comprehensive physical therapy evaluations and develop treatment plans
• Provide therapeutic exercises, gait training, and balance activities
• Educate patients and caregivers on safe mobility and fall prevention
• Monitor patient progress and adjust treatment plans as needed
• Coordinate care with physicians, nurses, and other healthcare team members
• Maintain accurate documentation in compliance with Medicare and regulatory standards
• Recommend durable medical equipment and home modifications

Required Qualifications

• Current Illinois Physical Therapist (PT) license in good standing
• Minimum 1 year of clinical PT experience (home health preferred)
• Valid driver's license and reliable transportation
• Current CPR certification
• Strong clinical reasoning and problem-solving skills
• Excellent communication and patient education abilities
• Proficiency with electronic medical records (EMR)

Preferred Qualifications

• DPT (Doctor of Physical Therapy) degree
• 2+ years of home health or geriatric PT experience
• OASIS-D1 certification
• Orthopedic or neurological specialty certification
• Bilingual (English/Spanish)

Work Schedule

Part-time, flexible scheduling (20-30 hours/week). Monday - Friday preferred, some weekend availability helpful. Typical caseload: 4-6 patients per day.
