Occupational Therapist (OT)

Job Overview

As an Occupational Therapist, you will deliver in-home OT services including ADL training, home safety assessments, adaptive equipment recommendations, and patient/caregiver education to support independence and quality of life.

Key Responsibilities

• Evaluate patient functional status and develop evidence-based treatment plans.
• Provide ADL/IADL training, fine motor therapy, and energy conservation techniques.
• Recommend and train on adaptive equipment and home modifications.
• Collaborate with PT, RN, SLP, MSW, and physicians for coordinated care.
• Document in EMR timely and accurately; track outcomes and progress.

Required Qualifications

• Current Illinois Occupational Therapy license (OTR/L).
• Degree in Occupational Therapy accredited program.
• 1+ years of clinical OT experience (home health preferred).
• Valid driver’s license and reliable transportation.
• Strong communication and patient education skills.

Work Schedule
