Job Overview
As an Occupational Therapist, you will deliver in-home OT services including ADL training, home safety assessments, adaptive equipment recommendations, and patient/caregiver education to support independence and quality of life.
Key Responsibilities
- Evaluate patient functional status and develop evidence-based treatment plans.
- Provide ADL/IADL training, fine motor therapy, and energy conservation techniques.
- Recommend and train on adaptive equipment and home modifications.
- Collaborate with PT, RN, SLP, MSW, and physicians for coordinated care.
- Document in EMR timely and accurately; track outcomes and progress.
Required Qualifications
- Current Illinois Occupational Therapy license (OTR/L).
- Degree in Occupational Therapy accredited program.
- 1+ years of clinical OT experience (home health preferred).
- Valid driver’s license and reliable transportation.
- Strong communication and patient education skills.
