Medical Social Worker (MSW)

Job Overview

As a Medical Social Worker, you will provide psychosocial assessments, counseling, and care coordination to home health patients and their families. You will connect patients with community resources, support advance care planning, and collaborate with our interdisciplinary team to promote safe transitions of care.

Key Responsibilities

• Conduct comprehensive psychosocial evaluations and develop care plans.
• Provide counseling, crisis intervention, and caregiver support.
• Coordinate resources such as Medicaid waivers, transportation, meal programs, and DME.
• Assist with advance directives, power of attorney, and end-of-life planning as appropriate.
• Collaborate with nurses, therapists, physicians, and community partners.
• Document timely and accurately within the EMR in compliance with regulations.

Required Qualifications

• Master’s degree in Social Work (MSW) from an accredited program.
• Current Illinois licensure (LSW/LCSW).
• 1+ years of experience in medical social work (home health, acute, or post-acute preferred).
• Knowledge of community resources and payer programs.
• Strong communication, advocacy, and documentation skills.

Work Schedule

Monday–Friday with occasional after-hours needs based on patient needs. Local travel required.
